II. Pre-Kindergarten Health Assessment and Immunization Compliance

Pursuant to NC General Statute 130A, each child shall have an annual health assessment and immunization records within 30 calendar days of their first enrollment date.

 

A.  Health Assessment - shall be done no more than 12 months prior to the date of school entry and shall be completed by one of the following:  NC Medical Board Licensed Physician; NC Certified Nurse Practitioner or Qualified NC Public Health Nurse.

 

B.  Immunization Requirements for 3-4 Year Old

  • 4 -   Diphtheria, Tetanus and Pertussis (DTap)
  • 3 -   Hepatitis B (Hep B)
  • 3-4  Haemophilus Influenzae Type b (Hib)
  • 1     Mumps, Measles, Rubella (MMR)
  • 3     Polio (Polio)
  • 3-4  Pneumococcal (PCV)
  • 1     Varicella (Var)
