E. Chronic Illness

The nurse will review the Permission for Emergency Medical Care and will note any condition the parent/guardian checks off/writes on the list of medical conditions.  Individual health plans will be sent home for the parent/guardian to complete and return them to your school nurse.   It is highly encouraged to discuss your child's health history with your school nurse in order to continue your child's care while in school.
